What to Expect
Your first visit to Baek Dental, PC is an important one as it introduces you to our practice and our friendly staff.  During the first visit, we will collect background information like medical history as well as take the time to get to know Dr. Baek.  Below you will find additional information to view before your first visit to Paul W. Appointments
In order to make your visit an easy process, please call our office to schedule your first appointment. Our friendly staff will assist you in getting everything set up.

Insurance
Our dental office accepts most Dental insurances, but we are not on any lists for DMO’s or HMO’s. If you have any questions about your dental insurance coverage, please contact our office and let our staff assist you.

Patient Forms
The below patient forms can be completed online before your first visit to expedite your check-in process. Please give our office a call after filling out the forms to book an appointment if you have not already done so.
